Roster Change Form

This document contains the Team Roster Change form.  This form is used to change your team roster if you have previously submitted one.  All roster changes must be received before February 11, 2006 by the Tournament Registrar.  No roster changes will be accepted during the team check-in and registration.  Roster changes should be mailed to:
